Obtaining copies of your medical records is easy and convenient. You can make your request in several different ways. You may request your medical records by:
• Visiting the Patient Portal at Royal Kiosks™ Patient Portal
Here you can access your patient portal or request an account.
• Downloading the authorization form below and faxing it to 720-874-4433, or emailing it to Medical.Records@riaco.com
The authorization form needs to be completely filled out before medical records can be released. Please note, incomplete forms cannot be processed.
After your request is received, the turnaround time is typically 5-7 business days. If this is an urgent request due to continuity of care, please call us directly at 720-493-3445.
Please be advised, images can be accessed through the patient portal but CANNOT be emailed.

Balance Verification
If you are our patient and have an upcoming appointment, you will need to request your medical images and reports from your previous facility. Please reach out to that facility to have your records sent over. If there is a delay in with us receiving your prior images, your appointment may be rescheduled for a later date.
